IMPORTANT INSTRUCTIONS :
1. उम्मीदवारों को आवेदन में सभी सूचनाएं सटीक भरनी हैं और सबमिट करने से पहले भलीभांति जांच लें, क्योंकि आवदेन सबमिट करने के बाद कोई बदलाव नहीं किया जा सकता।
2. उम्मीदवारों से अनुरोध है कि वह भुगतान करने के दिशानिर्देशों को भलीभांति पढ़ लें और भुगतान का स्क्रीनशॉट ले लें।
3. प्रतियोगिता परीक्षा का प्रश्न-पत्र हिन्दी एवं अंग्रेजी भाषा में कुल 400 अंकों का होगा, जिसमें बहुविकल्पीय वस्तुनिष्ठ प्रकृति के कुल 100 प्रश्न होंगे तथा प्रत्येक प्रश्न 4 अंकों का होगा। 
4. परीक्षा में गलत उत्तर के लिए नकारात्मक अंकन (Negative Marking) लागू होगा, जिसमें प्रत्येक गलत उत्तर हेतु 1 (एक) अंक काटा जाएगा।
5. मात्र रजिस्ट्रेशन करने या ऑनलाईन आवेदन शुल्क भुगतान करने से यह नहीं माना जाएगा कि आवेदक द्वारा पूर्ण रूप से ऑनलाईन आवेदन भर लिया गया है। केवल उन्हीं आवेदकों का CBT हेतु प्रवेश-पत्र (Admit Card) निर्गत किया जाएगा, जिनके द्वारा ऑनलाईन आवेदन पूर्ण रूप से निर्धारित आवेदन शुल्क के साथ जमा किया गया हो। आवेदन अपूर्ण या त्रुटिपूर्ण रहने तथा निर्धारित आवेदन शुल्क नहीं दिए जाने की स्थिति में आवेदक को प्रवेश पत्र निर्गत नहीं किया जाएगा।
6. आयोग सभी अभ्यर्थियों के लिए Computer Based Test के माध्यम से लिखित प्रतियोगिता परीक्षा आयोजित करेगा
7. आवेदकों को सलाह दी जाती है कि वे एक से अधिक आवेदन समर्पित न करें। एक से अधिक (Multiple) आवेदन प्राप्त होने की स्थिति में आवेदक की अभ्यर्थीता रद्द की जा सकती है।
8. अभ्यर्थी द्वारा ऑनलाईन आवेदन में भरे गये अपना नाम, जन्म तिथि, कोटि तथा अन्य सूचनाओं से सन्तुष्ट होने के उपरांत ही आवेदन शुल्क जमा करेंगे। सफल आवेदन समर्पित करने के उपरांत आवेदन शुल्क वापस नहीं किया जाएगा।

1. E-Mail ID (valid at least for one year).
2. Mobile No (valid at least for one year).
3. Scanned Copy of Recent Photograph (not more than 3 weeks old in jpg/jpeg format).
4. Scanned Copy of Signature with Black ink pen (in jpg/jpeg format).
5. Original Matriculation Certificate
6. Marksheets and original certificates of all years of MBBS or Provisional certificate from a recognized institute
7. Registration Certificate of Permanent Registration from National Medical Commission or State Medical Registration Council (with entry of all educational qualifications)
8. Original certificate of internship training / experience in a recognized hospital / institute
9. Caste certificate/Creamy layer free certificate/Economically weaker section certificate
10. Permanent Residence/Residential Certificate
11. Disability Certificate
13. Work Experience Certificate (in prescribed format)

चयन की प्रक्रिया – अभ्यर्थियों का चयन कोटिवार रिक्ति के अनुसार Computer Based Test (CBT) में प्राप्तांक के आधार पर किया जायेगा।
